Basis- und Zusatzversorgung in der gesetzlichen Krankenversicherung

Abstract

Obgleich lange Zeit tabuisiert, finden derzeit im wissenschaftlichen und politischen Bereich vielfältige Diskussionen um eine Anpassung des Leistungskataloges der gesetzlichen Krankenversicherung (GKV) an die gesellschaftlichen und wirtschaftlichen Veränderungen statt. Unbeschadet noch vorhandener beachtlicher Rationalisierungspotenziale dürften infolge der strukturellen Wachstumsschwäche der Finanzierungsbasis die Beitragseinnahmen bei Wahrung der Beitragssatzstabilität künftig kaum ausreichen, um die ausgabenseitigen Herausforderungen zu bewältigen. Es kann, um eine Verschärfung der impliziten Rationierung zu vermeiden, nicht um das Ob, sondern nur um das Wie von Reformen der GKV gehen. Even though it has been a taboo for a long time, at the moment manifold discussions in the social and political arena address the issue of adapting the benefits catalogue of Social Health Insurance (SHI) to social and economic changes. Irrespective of the still existing considerable rationalization potential the receipts from contributions – if the contribution rate is to remain constant – will hardly be sufficient to master the challenges caused by the expenditure side due to the structurally weak growth of the financing basis. In order to prevent an aggravation of the implicit rationing the crucial point to consider is the “how” – and not the “if” – of SHI reforms.
